Custom forms for events, surveys, product / payment / membership / booking

Accessing the Page

From the left main menu, click "Marketing", then select "Forms". Once inside, the top displays: Forms. The page is divided into a form list and a "Create Form" button. The list table includes columns for form title, form description, response count, and actions (preview / configure / responses / view online / copy link / delete).

Form Types

Question Types

After entering the form editor page, click the "Add Question" button to add questions. Each question can be configured with a "title", a "required" toggle, and "minimum/maximum answer options" (for multi-select types). Common question types:

Single-line text input
Radio Buttons
Customers can only select one option
Checkboxes
Customers can check multiple options; you can add an "Other" custom option
Dropdown Menu
Customers select one item from a dropdown menu
File Upload / Form Image Options
Customers upload files; use images as options
Date / Time / Date Range / Time Range
Customers select a date, time, or range
Phone Number / Number
Customers enter a phone number or numeric value
Custom URL / Custom Email
Customers enter a URL or email; the system performs format validation

Form Responses

Click the "Responses" button on any form to enter. You can search by keyword, filter by date range, sort from old/new to new/old, and "edit response" or "delete response" individually.

Form Sharing

View Form Online
Open the public link of the front-end form page; any customer can fill it out
Copy Link
Copy the form link to the clipboard
Set as Non-Fillable
Pause the form so customers can no longer submit
Operational Tips5-10 questions per form is ideal; longer forms reduce completion rates. For "Checkboxes", setting "minimum/maximum answer options" ensures data quality. For important data (phone, email), it is recommended to mark them as required.
